QUALIFICATIONS
Application Packets Must Include
(forms should be downloaded from desktop view)
- Completed MLMNS official application form
- Official transcript (must be received directly from Shelby Co. ATC administration)
- ACT or SAT score form
- Completed MLMNS Most Impactful Teacher form
- 250-350 word typed essay (click here for essay prompt)
~ Senior student enrolled in the Shelby County ATC pre-nursing program

~ Minimum GPA 3.0 in general and nursing curricula
​
~ Taken the ACT or SAT
​
~ Submitted an application for admission to a college or university in the state
of Kentucky with an accredited nursing program
Students will submit all components of the application packet to Shelby County ATC administration.
2025 Applications are due to the ATC by April 1, 2025
